Most training reports stop at attendance and satisfaction scores. Here's the case for reporting what actually changed, with two free training report templates: an Excel tracker that goes from attendance and test scores to on-the-job behavior, business results and payback, and a one-page Word report that leads with the conclusion.

Here’s an unpopular opinion: most training reports aren’t really reports. They’re attendance sheets with a logo on top.

You know the type. Forty-two people completed the course. Average satisfaction 4.6 out of 5. Ninety-four percent pass rate. Then a photo of people around a table. Every number on that page is true, and none of it tells the person paying for the training whether anything changed.

I’d like that to stop, so this post makes the case for a different kind of training report, plus two free templates built around it. The Excel tracker takes you from attendance to on-the-job results in one sheet. The Word report fits the whole story on one page, with the conclusion at the top.

Attendance is an input. Stop reporting it as an outcome.

Attendance tells you the training happened. That’s worth recording, and both templates do record it. But it’s the cheapest thing to measure, so it ends up as the headline. Then everyone acts surprised when the next budget review treats training as a cost with no return.

Satisfaction scores have the same problem. People enjoy a lot of training that changes nothing about how they work, and they can sit through dull training that changes plenty. A 4.6 is nice, but it isn’t evidence.

The structure I keep coming back to is the old four-level model from Donald Kirkpatrick, which has been around since the late 1950s. Not because it’s fashionable. It just asks the right questions in the right order:

Reaction: did they find it useful?

Learning: do they know more than before?

Behavior: are they doing anything differently at work?

Results: did the business number we cared about move?

Most training reports stop at level one, occasionally reach level two, and almost never touch three or four. Levels three and four are the only ones your boss actually cares about.

What a report looks like when it goes all the way

The sample in both templates is a made-up but realistic case. Ridgeway Home Services runs 24 heating and plumbing technicians. Too many jobs needed a second visit, usually because the van didn’t have the right part. So the company ran a “First-Time Fix” workshop in four small sessions over the summer. Here’s the whole story in one table:

Level

What was measured

Result

1 · Reaction

End-of-session survey

4.5 out of 5

2 · Learning

Same diagnostic test before and after

59 → 84 average; 20 of 22 passed

3 · Behavior

Supervisor ride-along 30 days later

17 of 22 using the new pre-visit checklist

4 · Results

Job data before vs after

First-time fix 71% → 79%; callbacks 9.4 → 6.8 per 100 jobs

Read that from top to bottom and you can see a chain. People liked it, they learned it, most of them started doing it, and the number moved. When the chain breaks, the report shows you where. If test scores jump but behavior doesn’t change, the problem is on the job, not in the classroom. In this case, it turned out the three technicians who weren’t using the checklist all worked from the same depot, with the oldest vans and nowhere to store the extra parts. No amount of retraining would fix that.

The Excel tracker

Training Report Tracker – Excel (.xlsx)

Download the Excel Training Report

The workbook has two tabs. On Participants you enter one row per person: team, session, whether they attended, pre-test and post-test scores, and what a supervisor saw 30 days later (Yes, No or Not yet). The gain and pass/fail columns calculate themselves.

The Training Report tab builds everything else from that. Each session gets its attendance and average scores. The four-level tiles read left to right. A results table compares the business measures before and after, and works out whether each one is improved, flat or worse based on which direction counts as better. At the bottom, a short cost section turns trainer fees and time off the job into a cost per person and a payback estimate. For Ridgeway, $11,402 all-in, about 30 avoided callbacks a month, and payback in roughly 2.3 months.

Notice that one measure in the sample didn’t move: average job time stayed at 1.9 hours. I left it in on purpose. A report that only shows the numbers that improved isn’t a report. It’s marketing. Flat results are useful too. Here, the flat job time shows that the extra prep didn’t slow technicians down.

The Word report

Training Report – Word (.docx)

Download the Word Training Report

This is the version for people who will read exactly one paragraph. That paragraph comes first, in a shaded box called The short version. Then the four-level table, what changed on the job (including what didn’t work), the cost, and four numbered recommendations. Everything fits on one page.

If you take one thing from the template, take the order. Conclusion first, evidence second, recommendations last. Most training reports run the other way round: background, agenda, attendance, feedback, and then the interesting bit on page three, where nobody reads it.

“But we can’t measure results for our training”

Sometimes that’s true. Compliance training, harassment prevention and onboarding don’t come with a neat before-and-after business metric. But you can almost always get to level three. Pick one or two behaviors the training was supposed to change, and check whether they’re happening a month later. It might be a manager observing, a quick audit or a short self-report. Any of them beats a satisfaction score.

And when you genuinely can’t measure the result, say so in the report. “Level 4 not measured: no reliable baseline” is honest. Leaving it out and hoping nobody asks isn’t.

A few things I’d do differently from most templates

Use the same test before and after. If the post-test is easier than the pre-test, the gain is fiction.

Choose the business metric before the training, not after. Otherwise you’ll go hunting for whichever number happened to improve.

Report the people who didn’t attend or didn’t pass. Two no-shows and two fails out of 24 is normal. Hiding them makes the rest look less credible.

Put a date on the follow-up. “Re-check in December” turns a one-off report into a habit.

The five questions a training report should answer

If you’d rather skip the templates and write your own, fine. Just make sure the finished report answers these five questions, in this order:

What problem was the training meant to fix? One sentence, with the number that showed the problem.

Did that number move? Before, after, and the change.

Are people actually doing the thing they were taught? Who is, who isn’t, and why not.

What did it cost, all in? Including the hours people spent away from their normal work, which is usually the biggest cost and the one most often left out.

What happens next? Repeat it, change it, extend it, or stop it.

A report that answers those five can be half a page long and still be more useful than a glossy twelve-page pack.

Where the training report fits

Training data feeds naturally into other reports. Individual results belong in each person’s performance report, especially where training was meant to close a specific gap. If your team tracks a handful of operational numbers every month, put the training’s target metric on your KPI report, so the improvement (or lack of one) stays visible after the training report is filed. If you’re assessing skills before deciding what training to run, our assessment report templates are the place to start.

Both files are free to edit and reuse. Delete Ridgeway’s numbers, put in your own program, and start with the Participants tab. Then try one thing for me: the next time someone asks how the training went, reply with level four first.

Two free timesheet templates: a weekly Excel timesheet that splits regular and overtime hours and works out gross pay, plus a client time log for freelancers, and a printable two-week timesheet form in Word. Here's which parts matter for employees, freelancers and small-business owners.

A timesheet looks like the simplest document in any business. It’s a grid of days and hours. But the person filling it in changes what it needs to do. An hourly employee needs overtime worked out correctly. A freelancer needs to know what to bill and who to bill it to. A small-business owner needs something people will actually fill in on time, and that will stand up if the Department of Labor ever asks.

So rather than one generic grid, this page has two free timesheet templates and a short guide to which parts matter for which person. The Excel file is a weekly calculator, plus a client time log on a second tab. The Word file is a printable pay-period timesheet for teams that still sign paper. Both are filled in with sample hours, so you can see them working before you clear them out.

Quick start: five minutes to your first timesheet

Download the Excel template and open the Weekly Timesheet tab.

Type the employee’s name, the Monday that starts the week, the hourly rate and your overtime threshold (40 is the US federal default) into the shaded cells.

Enter start, lunch and finish times for each day. Type 7:00 or 3:30 PM. Excel will understand either.

Read the bottom line. Hours, regular time, overtime and gross pay fill in on their own.

Print it or save it as a PDF, get it signed, and keep a copy. That’s the whole routine.

If you’d rather hand out paper, skip to the Word template further down. It works the same way, minus the math.

The Excel timesheet: what it does for you

Weekly Timesheet Template – Excel (.xlsx)

Download the Excel Timesheet

The sample week belongs to Jordan, a print technician who worked a long Wednesday, a late Friday and a Saturday morning. That came to 47.25 hours. The sheet keeps a running count of regular hours, so overtime kicks in at exactly the point Jordan crosses 40. Here, that’s partway through Friday, not on some arbitrary day. That’s how US federal overtime works: it’s based on hours in the workweek, not hours per day.

Line

How it’s calculated

Sample week

Hours per day

Finish minus start, minus the lunch gap

8.00 to 9.50

Regular hours

Running total, capped at the threshold you set

40.00

Overtime hours

Everything over the cap

7.25

Overtime pay

OT hours × rate × multiplier (1.5)

$244.69

Gross pay

Regular + overtime + PTO

$1,144.69

How the weekly sheet arrives at Jordan’s pay

A few details took some testing. A shift that finishes after midnight still calculates correctly, because the formula wraps the clock. A day with no lunch break is fine, so Saturday’s four hours don’t need dummy entries. The time cells reject nonsense like “7 o’clock”, so you won’t end up with a quietly broken total.

Which parts matter depends on who you are

This is where most timesheet advice goes generic. Here’s the same template seen from three chairs.

If you’re an hourly employee

Your timesheet is your evidence. If your paycheck looks short, it’s the only document you control. Three habits make it bulletproof:

Fill it in daily, not on Friday afternoon. A week of guessed start times is how 15 minutes a day goes missing. That’s over 60 hours a year.

Record the real lunch, not the scheduled one. If you ate at your desk and answered calls, that time isn’t an unpaid break. Under federal rules, short breaks of about 5 to 20 minutes are paid time anyway.

Keep your own copy. Save the PDF before you send it. If there’s a dispute later, a copy dated the week it happened is worth a lot more than memory.

Check the overtime column every week. If you’re non-exempt and it says 7.25 while your pay stub says 5, that’s a conversation worth having.

If you’re a freelancer or contractor

You probably don’t care about overtime. You care about two numbers: billable hours and your real hourly rate. That’s what the second tab, Client Time Log, is for. Every block of work gets one line: client, task, start, finish and whether it’s billable. The sheet then totals hours and fees for each client.

The sample tells a familiar story. A designer logs 15.75 hours across three clients in a week, but only 14.25 of them can be billed. The rest went on admin and a re-export that was the designer’s own mistake. Their quoted rates run from $60 to $70, but the effective rate across every hour worked comes out at $59.21. Watch that number over a month. It tells you which clients are actually worth it.

Track non-billable time too, even though nobody pays for it. If you only log billable hours, you’ll keep underquoting, because you’ll never see how much invoicing, revisions and email take.

If you run a small team

Your problem isn’t formulas. It’s getting timesheets back on time, complete and signed. What helps:

Pick one workweek and stick to it. The template assumes Monday to Sunday. If your payroll week runs Sunday to Saturday, change the start date and the rest follows.

Set the overtime rules once. The threshold and multiplier sit in two cells at the top. If you’re in a state with daily overtime, such as California, you’ll need a daily rule on top of the weekly one. The template only handles the weekly federal rule.

Make approval a real step. There’s a manager signature line for a reason. An unapproved timesheet that went straight to payroll is a weak record.

Keep them. Federal rules expect payroll records to be kept for three years, and time cards and wage-calculation records for two. A folder of signed PDFs per pay period covers it.

Once timesheets are in, they feed straight into the rest of the payroll paperwork. Our payroll report template picks up where this one stops, and the attendance report template is better for spotting absence patterns across a whole team than a stack of individual sheets.

The Word timesheet: for paper, clipboards and signatures

Printable Timesheet Form – Word (.docx)

Download the Word Timesheet

Some workplaces simply run on paper. Think of a shop counter, a job site trailer, or a volunteer desk. This version is a two-week pay-period form with a row for every day, a notes column and a summary strip at the bottom. In the sample, Aisha worked 25 hours in week one and 21 in week two. That’s 46 hours, or $793.50 at $17.25 an hour, and no overtime because neither week went over 40.

That last point trips people up. A two-week period with more than 80 hours doesn’t automatically mean overtime, and one with fewer than 80 doesn’t rule it out. Overtime is counted week by week. A 50-hour week followed by a 30-hour week still owes 10 hours of overtime, even though the period total is exactly 80. The Word form keeps the weeks separate on purpose so that this is obvious at a glance.

Because it’s a normal Word table, you can add a column for job codes, rename “Unpaid break” to suit your policy, or switch the period to weekly by deleting the second block.

Small things that keep timesheets honest

Pick a rounding rule and write it down. Rounding to the nearest quarter hour is common and allowed in the US, as long as it evens out over time and doesn’t always favor the employer. Rounding 7:07 to 7:15 and 4:53 to 4:45 every day isn’t neutral.

Don’t let anyone pre-fill the week. A timesheet showing exactly 8.00 hours every day for a month is usually fiction, and it protects nobody.

Use the notes column. “Covered for Dev” or “inventory count” takes four seconds and answers a question a manager would otherwise have to ask.

Store the signed copy, not the spreadsheet. A spreadsheet can be changed later. A signed PDF from the week in question is the actual record.

Which template should you pick?

You are…

Start with

Why

Hourly employee tracking your own time

Excel: Weekly Timesheet tab

Overtime and pay are worked out the way payroll should do them

Freelancer or contractor

Excel: Client Time Log tab

Billable hours, fees per client and your real hourly rate

Manager with paper-based staff

Word: pay-period form

Prints cleanly, signs easily, weeks kept separate for overtime

Manager with staff on laptops

Excel, one file per person per week

Totals and pay calculate as soon as the hours go in

For a weekly summary of what the team actually got done, rather than how many hours it took, pair these with the weekly report templates. Hours tell you what work cost. A weekly report tells you what it produced.

Both timesheet files are free to use, edit and share inside your business. Clear the sample hours, put in your own company name, and save a blank copy as your master before the first real week goes in.

A turnover rate on its own tells you very little. “We were at 19% last year” sounds like a fact, but it hides the questions that actually matter. Who left? How long had they been with you? Did you want to keep them? Did they quit, or did you let them go?

A good employee turnover report answers those questions on one or two pages, and it does the math the same way every month so the trend means something. This guide walks through how to calculate turnover properly, which numbers deserve a place next to the headline rate, and how to read the result without overreacting to it. You can download a free employee turnover report template in Excel and Word along the way. Both files come filled in with a year of sample data from a fictional home health agency so you can see the finished report before you add your own.

Start with the formula (and the two places people get it wrong)

The standard calculation is simple:

Turnover rate = number of separations in the period ÷ average headcount in the period × 100

A separation is anyone who left the payroll for any reason: resignations, terminations, layoffs and retirements. Most of the arguments start with the two inputs.

Mistake 1: dividing by today’s headcount

If you divide twelve months of exits by the headcount on the last day of the year, a company that grew will look better than it is and one that shrank will look worse. Use an average instead. The cleanest version is the average of each month’s (opening headcount + closing headcount) ÷ 2, then the average of those twelve figures. That’s what the Excel template does in the Average headcount column.

Here’s the sample company’s year in short form:

Period

Opening headcount

Hires

Exits

Closing headcount

Average headcount

Turnover

Oct 2025 – Sep 2026

210

49

41

218

213.75

41 ÷ 213.75 = 19.2%

Divide by the closing figure of 218 instead and you get 18.8%. Small difference here, but at a company that hired fast or ran a layoff it can be several points.

Mistake 2: annualizing a single bad month

One month of 2.4% turnover is not “29% a year.” Monthly figures jump around, especially in small teams, so treat them as a signal to look closer rather than as a forecast. If you do report a quarter, say clearly whether the number is quarterly (Northfield’s Q3 was 5.5%) or annualized (about 22% at that pace). Mixing the two in one slide is how a board ends up debating the wrong figure.

A third, smaller point: decide up front whether to include temporary staff, interns and internal transfers. Most HR teams leave out transfers and seasonal workers. Either way, write the rule in the report so next year’s numbers can be compared fairly.

Four numbers that matter more than the headline rate

The overall rate is a starting point. These four tell you what’s behind it, and all of them are calculated for you in the template.

Voluntary turnover

Resignations only, divided by the same average headcount. This is the number most people mean when they say “turnover,” and it’s the one you can do the most about. In the sample, 31 of the 41 exits were voluntary, so voluntary turnover is 14.5%. The other 10 were terminations for attendance, performance or policy reasons. Those deserve attention too, but they say more about hiring and onboarding than about why people choose to leave.

First-year turnover

The share of leavers who had been with you less than a year. This one is often the most useful number in the whole report. At Northfield it’s 61%: 25 of the 41 people who left hadn’t reached their first anniversary. When most of your exits come from new hires, the fix usually lies in recruiting, pay at the entry level and the first 90 days, not in engagement surveys for long-serving staff.

Regrettable losses

A plain count of the leavers you would have hired back tomorrow. It’s a judgment call made by the manager, and that’s fine. Twenty-two regrettable exits tells a very different story from 41 total. If your best performers are leaving, the headline rate can look healthy while the organization is quietly getting weaker.

Retention rate

Turnover’s mirror image, but not exactly the opposite. Retention asks: of the people employed on day one of the period, what share are still here at the end? New hires who joined and left within the year don’t count. In the sample, 26 of the 210 opening employees left, so retention is (210 − 26) ÷ 210 = 87.6%. The Excel file works this out with a COUNTIFS that looks for leavers whose hire date is before the period start.

The templates

Two files, one dataset. Use the spreadsheet as the working tool that HR updates every month. Use the Word document when the numbers need to go to leadership with some explanation attached.

Excel: Employee Turnover Report with Leavers Log

The workbook has four tabs:

Turnover Summary: five headline figures at the top (turnover, voluntary turnover, retention, first-year leavers and regrettable losses), a twelve-month table, and breakdowns by exit reason and by tenure at exit.

By Department: exits, voluntary and involuntary split, turnover rate, first-year exits, average tenure of leavers and regrettable losses for each team. Any department running above the company rate is highlighted.

Leavers Log: one row per person who left, with drop-downs for department, separation type, exit reason and the regrettable flag. Tenure in years and a tenure band are calculated from the hire and exit dates.

Lists: the departments and exit reasons that feed the drop-downs. Rename them to match your organization.

You only type in three places: the period start date and opening headcount, the number of hires each month, and the Leavers Log. Monthly exits are counted from the log with
COUNTIFS
on exit date and type, closing headcount is opening + hires − exits, and every rate follows from there. Months running more than 25% above the average monthly rate are shaded, and two small checks warn you if a leaver has no exit reason or if the department totals don’t match the summary.

Employee Turnover Report – Excel (.xlsx)

Word: Quarterly Employee Turnover Report

A two-page write-up for a leadership meeting, built on the same data. It opens with a short plain-language summary and four headline figures, then compares this quarter with the last one, shows turnover by department, lists the main exit reasons with an anonymized quote from an exit interview, sets out the actions being taken with owners and due dates, and ends with a short watch list for next quarter and a sign-off block. Replace the sample text with your own. The structure is the useful part.

Quarterly Employee Turnover Report – Word (.docx)

Reading a real report: what the Northfield numbers say

Numbers only help if someone draws a conclusion from them. Here’s how I’d read the sample data if it landed on my desk.

The headline is fine. The detail isn’t. 19.2% annual turnover is unremarkable for a home care business, where aide roles turn over constantly. Stopping there would miss the point, though. The department view shows where the exits are concentrated:

Department

Avg headcount

Exits

Turnover

Exits in first year

Home Care Aides

92

21

22.8%

16

Field Nursing

56

10

17.9%

5

Therapy

23

3

13.0%

0

Scheduling & Intake

19

4

21.1%

2

Administration

24

3

12.5%

2

Aides make up 43% of the workforce but account for half of all exits and 16 of the 25 first-year leavers. That one group is the story.

The quarter got worse, and in a specific way. Q3 had 12 exits against 9 in Q2. Eleven of the twelve were voluntary and eleven had less than a year of service. When the same pattern shows up in both columns, it usually points to one cause rather than several.

The exit reasons line up with the tenure data. Pay or a better offer is the top reason (10 exits), followed by schedules and hours (7). All four “career growth” exits were aides leaving to study nursing. Put together: newer aides are leaving early, for slightly better pay and more predictable schedules. That’s something the business can act on, which is why the Word report pairs each finding with an action and an owner.

Small teams need a footnote. Scheduling & Intake shows 21.1%, which looks alarming until you see it’s four people on a team of nineteen. The template highlights it because it’s above the company rate. A human still has to decide whether it matters.

Watch the regrettable count in Field Nursing. Ten exits, seven of them regrettable. Nurses are harder to replace than aides, so a lower turnover rate can still hurt more. That’s why regrettable losses get their own column.

Should you compare yourself to an industry benchmark?

Carefully. Published benchmarks vary a lot depending on who collected them, which separations they count and how they define headcount. In the United States, the Bureau of Labor Statistics publishes monthly quits and total separation rates by industry in its JOLTS survey, and many industry associations run their own surveys. Both are worth a look for context.

The comparison that matters most is with yourself: the same calculation, run the same way, every month. If your first-year turnover drops from 61% to 45% after you change onboarding, that tells you far more than learning you’re three points above some sector average. If you do quote a benchmark, put the source and the year next to it, and make sure you’re comparing like with like, such as voluntary with voluntary or annual with annual.

Monthly, quarterly or once a year?

All three, at different levels of detail.

Monthly: update the Leavers Log and the hires column. Takes ten minutes if exits are logged as they happen. Look at the shaded months and the department view, and leave it there unless something stands out.

Quarterly: write the Word report. Compare the quarter with the one before, pick out two or three findings, and agree actions with owners. Most leadership teams don’t need more often than this.

Annually: run the full twelve-month view, review your exit reason list (rename anything vague, split “Other” if it’s grown), and reset targets.

Turnover rarely tells the whole people story alone. It sits well next to an employee performance report, since you’ll want to know whether the people leaving were strong or weak performers, and an attendance report, because rising absence often comes a few months before resignations. If you track people measures alongside business ones, a KPI report can carry the headline turnover and retention rates each month.

Getting exit reasons you can trust

The exit reason column is the weakest part of most turnover reports, so it’s worth a few minutes of extra care.

Keep the list short. Eight to twelve reasons is plenty. If one reason grows past about a quarter of exits, split it into more specific ones.

Separate the stated reason from the real one. “Personal reasons” on a resignation letter often turns into “my manager” in a candid exit conversation. Record the reason you believe is true and note how you know.

Track exit interview completion. The template shows it at 77% of voluntary leavers. Below about 70%, your reasons breakdown is based on too few people to lean on.

Ask someone other than the direct manager. People are more open with HR, or with a short anonymous form, than with the boss they’re leaving.

Keep the log factual, the way you would in an employee incident report: dates, roles and reasons, not opinions about the person. Some of what lands in it is sensitive, so store the workbook where only HR and the people who need it can open it, and share the department and company totals rather than the named list.

One last thing

The most useful turnover reports I’ve seen are short. They show a rate calculated the same way every time, the three or four figures that explain it, and a sentence or two about what the team will do next. The template takes care of the calculations. The judgment about what the numbers mean is still yours, and it’s the part leadership will actually remember.

Download a free payroll report template in Excel and Word. Calculate regular and overtime pay, gross pay, taxes, 401(k) and health deductions and net pay for every employee automatically, see payroll cost by department and taxes to remit, and turn the results into a ready-to-approve payroll summary report.

A payroll report template gives you one clear, repeatable record of what every employee earned in a pay period, what was withheld from their pay, and what the payroll actually cost your business. Instead of piecing together timesheets, pay stubs and tax notices, a payroll report lays out hours, gross pay, taxes, deductions and net pay line by line, then totals everything so you know exactly how much to fund, deposit and record.

Below you can download two free, fully editable templates. The first is an Excel payroll report that works as a payroll register: it calculates regular pay, overtime, gross pay, 401(k), federal and state income tax, Social Security, Medicare and net pay for each employee, then rolls the results into a department summary and a tax liability schedule. The second is a Word payroll summary report you can send to an owner, controller or accountant: it presents the headline totals, payroll by department, an earnings and deductions breakdown, the taxes to remit and an approval block on a clean, printable page.

What Is a Payroll Report?

A payroll report is a summary of employee compensation for a specific pay period, such as a week, two weeks or a month. For each employee it shows hours worked, pay rate, gross earnings, every tax and deduction taken out, and the final net pay. At the bottom it totals those amounts and usually adds the employer’s share of payroll taxes, which gives you the full cost of the payroll.

Most payroll reports have two levels. The detail level, often called the payroll register, lists every employee on its own row. The summary level groups the figures by department, location or pay type and lists the tax and benefit amounts that must be paid to government agencies, insurers and retirement plans. Together they answer three questions: what did we pay, what do we owe, and where did the cost go?

A payroll report is also a control document. Gross pay minus total deductions must equal net pay, net pay must match the amount that leaves your bank account, and the withholding totals must match your tax deposits and quarterly filings.

Why Use a Payroll Report?

Payroll is usually one of the largest expenses a business has, and mistakes are costly for both employees and employers. A consistent payroll report helps you:

Pay people correctly. Seeing hours, rates, overtime and deductions side by side makes it easy to catch a missing timesheet or a wrong rate before payday.

Fund payroll and plan cash. Net pay plus tax and benefit remittances tells you exactly how much cash you need and when it leaves the account.

Stay on top of tax deposits. The liability totals show how much federal income tax, Social Security, Medicare and state tax to deposit and by which date.

Record accurate journal entries. Gross wages, employer taxes and each withholding liability map directly to accounts in your general ledger.

Control labor costs. Department totals and overtime figures reveal where payroll is growing and whether overtime is becoming routine.

Support audits and filings. Payroll reports back up quarterly Form 941 filings, year-end W-2s, workers’ compensation audits and lender requests.

Free Payroll Report Templates (Editable)

Both templates use the same sample company and the same September 14–27, 2026 biweekly pay period, so you can see how the detailed spreadsheet and the written summary fit together.

1. Payroll Report (Excel)

This workbook is the working tool. On the Payroll Register sheet you set the pay period, pay date, pay periods per year, overtime multiplier and tax rates in the yellow cells, then enter one row per employee: name, department, pay type (Hourly or Salary), pay rate, regular hours, overtime hours, bonus, 401(k) percentage, pre-tax health insurance and federal withholding rate. Regular pay, overtime pay, gross pay, every deduction and net pay calculate on their own, and cards at the top show total gross pay, total net pay and total payroll cost.

The Payroll Summary sheet uses SUMIFS and COUNTIF to total headcount, hours, gross pay, pre-tax deductions, employee taxes, net pay and employer FICA for each department, with a bar chart of payroll cost. A tax and deduction liability table shows the employee share, employer share and total to remit for each item, and a built-in check confirms that gross pay minus deductions equals net pay.

Payroll Report – Excel (.xlsx)

Best for: small business owners, bookkeepers and office managers who run payroll in a spreadsheet or want to double-check the figures from their payroll software.

2. Payroll Summary Report (Word)

This document is the reporting layer. It opens with a company header, pay period, pay date, pay frequency and preparer, then gives a short executive summary and four headline figures: gross pay, net pay, employer taxes and total payroll cost. A department table shows headcount, hours, gross pay, net pay and total cost for each team.

An earnings and deductions table walks from regular, overtime and bonus pay down to net pay, with each item as a percentage of gross. The second page lists every tax and deduction liability with the employee share, employer share, total to remit and due date, followed by notes that explain unusual items and an approval table for the preparer, reviewer and approver.

What to Include

Whatever tool you use, a complete payroll report contains the following sections. The first group comes from timesheets and employee records; the rest is calculated.

Section

What It Shows

Why It Matters

Report header

Company name, pay period dates and pay date

Ties every figure to one specific payroll run

Employee details

Name, employee ID or department, pay type

Lets you total and review pay by team or cost center

Pay rate

Hourly rate or annual salary

The basis for every earnings calculation

Hours worked

Regular hours and overtime hours

Drives hourly pay and flags excessive overtime

Gross pay

Regular pay plus overtime, bonuses and commissions

The starting point for taxes, deductions and expense

Pre-tax deductions

401(k), health insurance and other benefit deductions

Reduce taxable wages and must be sent to providers

Employee taxes

Federal and state income tax, Social Security, Medicare

Amounts withheld that you must deposit on time

Net pay

Gross pay minus all deductions

The amount actually paid to each employee

Employer taxes

Employer Social Security and Medicare (and FUTA/SUTA)

Part of the true cost of payroll

Totals and summaries

Totals by department and by liability

Support cash planning, journal entries and filings

Notes and approval

Explanations, reviewer and approver sign-off

Creates an audit trail for every payroll run

How to Create a Payroll Report (Step by Step)

Set up the pay period. Enter the pay period start and end dates, the pay date and the number of pay periods per year (26 for biweekly, 24 for semi-monthly, 52 for weekly, 12 for monthly).

Confirm your tax rates. Check the Social Security and Medicare rates, your state income tax rate and the overtime multiplier in the settings cells.

Collect approved timesheets. Gather regular and overtime hours for hourly employees, plus any bonuses, commissions or other pay for the period.

Enter employee details. Add each employee’s department, pay type, rate, hours, bonus, 401(k) percentage, health insurance premium and federal withholding rate in columns A to J.

Let the formulas calculate pay. The template works out regular pay, overtime pay, gross pay, each deduction and net pay. Scan the overtime column, which is highlighted when hours are entered.

Review the totals. Check that gross pay minus total deductions equals net pay and that the Payroll Summary check shows Balanced.

Schedule the remittances. Use the liability table to plan federal tax deposits, state withholding payments, 401(k) transfers and insurance premiums.

Prepare the summary report. Copy the headline totals, department figures and liabilities into the Word template, add notes on unusual items and send it for approval.

File and compare. Save a copy of every payroll run. Comparing periods shows trends in overtime, headcount and labor cost.

Formulas and Example

The Excel template keeps its logic simple so you can audit every number. Settings sit in the yellow cells (periods per year in E4, overtime multiplier in H3, state tax rate in H4, Social Security in J3 and Medicare in J4) and the first employee is in row 7. Each formula is copied down the table.

Column

Formula (row 7)

What It Does

Regular Pay

=ROUND(IF(C7="Salary",D7/$E$4,D7*E7),2)

Salary ÷ periods per year, or hourly rate × regular hours

Overtime Pay

=IF(C7="Hourly",ROUND(D7*F7*$H$3,2),0)

Hourly rate × overtime hours × 1.5 (salaried staff get 0)

Gross Pay

=K7+L7+G7

Regular pay plus overtime plus bonus

401(k)

=ROUND(M7*H7,2)

Gross pay × the employee’s contribution percentage

Federal Tax

=ROUND(MAX(0,M7-N7-I7)*J7,2)

Taxable wages (after 401(k) and health) × withholding rate

Social Security

=ROUND(MAX(0,M7-I7)*$J$3,2)

FICA wages (after pre-tax health) × 6.2%

Medicare

=ROUND(MAX(0,M7-I7)*$J$4,2)

FICA wages × 1.45%

Net Pay

=M7-S7

Gross pay minus total deductions

Department total

=SUMIFS('Payroll Register'!$M$7:$M$18,'Payroll Register'!$B$7:$B$18,$A7)

Adds gross pay for the department named in A7 of the summary

Example: Linda Okafor is an hourly employee in Field Services paid $31.00 an hour. She worked 80 regular hours and 9 overtime hours. Regular pay is 80 × $31.00 = $2,480.00 and overtime pay is 9 × $31.00 × 1.5 = $418.50, so gross pay is $2,898.50. Her 5% 401(k) contribution is $144.93 and her health premium is $210.00. Social Security is ($2,898.50 − $210.00) × 6.2% = $166.69 and Medicare is $38.98. Federal tax at 10% of $2,543.57 taxable wages is $254.36 and state tax at 4% is $101.74. Total deductions are $916.70, leaving net pay of $1,981.80.

Across all 12 sample employees, gross pay is $33,513.75: $28,186.99 regular pay (84.1%), $1,626.76 overtime (4.9%) and $3,700.00 bonuses (11.0%). Deductions total $9,853.24, or 29.4% of gross, so net pay is $23,660.51. Adding $2,442.18 of employer Social Security and Medicare brings the total payroll cost to $35,955.93, and the business must remit $12,295.42 in taxes and benefit deductions.

Note: the template uses a simple effective federal withholding rate per employee so the math is easy to follow. For real payroll, take federal withholding from IRS Publication 15-T or your payroll software, and remember that Social Security stops at the annual wage base and that FUTA, SUTA and local taxes may also apply.

Tips

Lock the cutoff dates. Pick a fixed timesheet cutoff for each pay period so late hours are always handled the same way in the next run.

Review overtime every run. Overtime that keeps appearing for the same team usually points to understaffing or scheduling issues, not a one-off rush.

Separate pre-tax and post-tax deductions. Health insurance under a Section 125 plan reduces both income tax and FICA wages, while traditional 401(k) contributions reduce only income tax wages. Mixing them up changes every tax figure.

Reconcile to the bank. Net pay on the report should match the direct deposit file and the bank debit to the cent.

Keep a payroll calendar. List pay dates, tax deposit dates, quarterly filing dates and benefit remittance dates in one place so nothing is paid late.

Protect the file. Payroll reports contain sensitive pay information. Store them in a restricted folder and share the summary report rather than the full register when possible.

Insert rows inside the table. Add new employees above the TOTAL row so every total and summary formula expands to include them.

Payroll Report vs Payroll Register vs Pay Stub

These three documents are related but serve different readers. The table below shows how they compare.

Feature

Payroll Report

Payroll Register

Pay Stub

Who it is for

Owners, managers, accountants

Payroll and accounting staff

The individual employee

Level of detail

Totals by department and liability

Every employee, every earning and deduction

One employee’s pay for one period

Main purpose

Review, approve and plan cash

Calculate and record payroll

Explain an employee’s pay

Shows employer taxes

Yes

Often

Usually not

Typical use

Management reporting, journal entries

Audit trail, tax filings, reconciliations

Employee records, proof of income

In these templates

Word payroll summary report

Excel Payroll Register sheet

Not included

Frequently Asked Questions

What is the difference between gross pay and net pay?

Gross pay is the total an employee earns before anything is taken out, including regular pay, overtime, bonuses and commissions. Net pay, often called take-home pay, is what is left after income taxes, Social Security, Medicare and benefit deductions such as 401(k) and health insurance. In the sample payroll, net pay is 70.6% of gross pay.

How often should I run a payroll report?

Run a payroll report for every pay period, before payday, so the figures can be reviewed and approved. Many businesses also prepare monthly, quarterly and year-to-date payroll summaries to support journal entries, Form 941 filings and year-end W-2 preparation.

What is the total cost of payroll for an employer?

The true cost of payroll is gross pay plus the employer’s share of payroll taxes and any employer-paid benefits. At minimum, employers match Social Security (6.2%) and Medicare (1.45%), and most also pay federal and state unemployment taxes. The Excel template adds employer FICA to gross pay to show total payroll cost, and you can extend it with columns for unemployment tax or employer benefit contributions.

Can I use this template to calculate federal withholding?

The template uses a simplified effective withholding rate for each employee so you can see how the numbers flow. Actual federal withholding depends on each employee’s Form W-4, pay frequency and the IRS tables in Publication 15-T. Calculate the real amount with those tables or your payroll software and enter it in place of the rate-based figure.

Can I use these templates in Google Sheets or Google Docs?

Yes. Upload the .xlsx file to Google Drive and open it with Google Sheets; the IF, ROUND, SUMIFS and COUNTIF formulas work as they do in Excel, though some formatting such as data bars may look different. The .docx report opens in Google Docs, Word and LibreOffice Writer.

An employee incident report is an official HR and safety document used to record details of an unexpected workplace event—such as an injury, illness, property damage, near miss, or behavioral infraction. It serves as a factual, chronological account of what happened, who was involved, where the event took place, and what immediate actions were taken.

Core Components of a Complete Incident Report

To ensure complete documentation and legal compliance, a standard employee incident report form must contain:

General Information: Date, time, exact location of the incident, and full names/job titles of the involved employee and reporting supervisor.

Detailed Incident Description: A factual, step-by-step account of what occurred leading up to, during, and immediately after the event (avoiding subjective opinions).

Injury or Damage Assessment: Specific details regarding physical injuries sustained, medical treatment rendered (first aid vs. hospital care), or physical property damaged.

Witness Statements: Contact details and brief, signed accounts from individuals who observed the incident.

Root Cause Analysis & Corrective Actions: Identification of contributing factors (e.g., equipment failure, lack of PPE, spill) and actionable preventive steps assigned to responsible managers.

Formal Signatures: Dated sign-off blocks for the affected employee, investigating supervisor, and HR/Safety Officer.

Benefits of Using Standardized Incident Templates

Download a free employee attendance report template: an Excel attendance sheet with dropdown codes, automatic totals and attendance %, plus a Word monthly attendance report for HR and management.

An employee attendance report template helps you record who was present, absent, on leave, sick or late every working day, and turns those daily marks into clear monthly totals and attendance percentages. Instead of chasing paper registers or messy spreadsheets, HR teams, managers and small business owners can see attendance patterns at a glance and act before absenteeism becomes a problem.

On this page you can download two free, fully editable templates that work together: an Employee Attendance Sheet in Excel with a dropdown for every day of the month, automatic totals and attendance % for each employee, and a Monthly Attendance Report in Word that summarizes attendance by department, absence reasons and employees who need follow-up – ready to send to management.

What Is an Employee Attendance Report?

An employee attendance report is a document that tracks the presence and absence of employees over a period – usually a week or a month. It shows the number of days each employee worked, the days they were absent and why (annual leave, sick leave, unpaid absence), late arrivals, half days and an overall attendance rate. The report gives HR and payroll accurate data and gives managers insight into workforce availability.

Attendance data also feeds into other HR documents. It is a key input for an employee performance report, it supports payroll and leave balances, and attendance rate is a common metric in a KPI report or monthly business report.

Why Track Employee Attendance?

Consistent attendance tracking protects both the business and its employees. A good attendance template helps you to:

Pay people correctly – accurate working days, leave and half days for payroll.

Reduce absenteeism by spotting frequent or patterned absences early.

Plan staffing so shifts and projects are covered when people are away.

Manage leave balances for annual leave and sick leave.

Treat employees fairly with objective records instead of impressions.

Meet legal and audit requirements for working time and leave records.

Support performance reviews with reliable attendance history.

Free Employee Attendance Report Templates (Editable)

1. Employee Attendance Sheet Template (Excel)

This monthly employee attendance sheet in Excel is built for daily tracking. Enter the month and year once and the sheet automatically fills in the dates and weekdays for the whole month, with weekends highlighted. For each employee, choose an attendance code from the dropdown in each day cell: P (present), A (absent), L (leave), S (sick), HD (half day), LT (late), H (holiday) or WO (weekly off). Each code is colour-coded so the month is easy to read.

Totals are calculated automatically for every employee: days present, absent, leave, sick, half days, late arrivals, working days and attendance %. Anyone below 90% attendance is highlighted in red. A monthly summary shows the average attendance rate and the number of employees below target, and a bar chart compares attendance across the team.

2. Monthly Attendance Report Template (Word)

This monthly attendance report template in Word turns raw attendance data into a summary for management. It starts with report details – company, department, reporting period, prepared by and date – followed by an overview of headcount, working days, average attendance rate, total absence days and late arrivals.

Further sections break down attendance by department, absence by reason (annual leave, sick leave, unpaid leave, unauthorized absence), employees requiring attention, late arrivals and early departures, and observations with recommended actions. A sign-off block for the HR manager and department head completes the report on a single page.

Best for: HR managers and department heads who need to report attendance trends to senior management each month.

What to Include in an Attendance Report

Section

What it includes

Why it matters

Employee details

Name, employee ID, department

Identifies each record

Daily attendance

Status code for every working day

The source data for all totals

Absence types

Leave, sick, unpaid, unauthorized

Separates planned from unplanned absence

Late arrivals / half days

Count of late days and half days

Highlights punctuality issues

Totals and attendance %

Days present, days absent, rate

Feeds payroll and KPIs

Department summary

Average rate per team

Shows where absence is concentrated

Follow-up actions

Employees below target, next steps

Turns data into action

Approval

HR and manager sign-off

Confirms the record is accurate

How to Create an Employee Attendance Report (Step by Step)

Set the period. Choose the month and year in the Excel sheet so dates and weekdays update automatically.

Add your employees. Enter names, IDs and departments in the employee columns.

Mark weekends and holidays. Use WO for weekly offs and H for public holidays so they are not counted as working days.

Record attendance daily. Select a code for each employee every day – recording daily is far more accurate than filling in at month end.

Check the totals. Review days present, absences and attendance % for each employee.

Summarize in the report. Copy the key figures into the Word monthly attendance report and complete the department and absence-reason tables.

Follow up. List employees below target, note reasons and agree actions, then sign off and share the report.

Attendance Rate and Absenteeism Formulas

These standard formulas are used in the templates and are useful for any attendance analysis:

Working days = Days in month − Weekly offs − Holidays

Attendance rate % = (Days present + 0.5 × Half days) ÷ Working days × 100

Absenteeism rate % = Days absent ÷ Working days × 100

Unplanned absence % = (Sick days + Unauthorized absences) ÷ Working days × 100

Average attendance = Sum of employee attendance rates ÷ Number of employees

Example: an employee with 22 working days who was present 19 days and took 1 half day has an attendance rate of (19 + 0.5) ÷ 22 = 88.6%, which is below a 90% target and would be flagged for a conversation. In the sample data in the Excel template, the team average is 93.0%, with 4 employees below 90%.

Tips to Improve Employee Attendance

Publish a clear attendance and leave policy so everyone knows the rules.

Record attendance every day and keep codes consistent.

Look for patterns, such as frequent Monday or Friday absences, rather than single days.

Hold return-to-work conversations after unplanned absences.

Recognize and reward good attendance.

Offer flexible working where possible to reduce unplanned absence.

Document serious issues with an incident report and review them in the employee’s assessment.

Daily, Weekly or Monthly Attendance Reports?

Small teams and shift-based businesses often check attendance every day as part of a daily report, and supervisors may summarize it in a weekly report. For HR and payroll, the monthly attendance report is the standard because it matches the payroll cycle and makes trends easy to see. Many organizations also roll monthly figures into a quarterly or annual business report to track absenteeism over time.

Frequently Asked Questions (FAQ)

What is an employee attendance report template?

An employee attendance report template is a ready-made Excel or Word document for recording employee presence, absences, leave, sick days and late arrivals, and calculating totals and attendance percentages for a period such as a month.

How do I make an attendance sheet in Excel?

Put employee names in rows and the days of the month in columns, add a dropdown list of attendance codes using Data Validation, then use COUNTIF formulas to total each code and calculate attendance %. The free Excel template on this page already includes automatic dates, dropdowns, colour coding, totals and a chart.

How do you calculate attendance percentage?

Divide the number of days present by the number of working days and multiply by 100. If you count half days, add half of them to days present first. For example, 20 days present out of 22 working days is 90.9%.

What is a good employee attendance rate?

Many organizations aim for an attendance rate of 95% or higher, and treat anything below 90% as a level that needs a conversation. The right target depends on your industry, leave policy and how holidays are counted.

What codes are used in an attendance register?

Common codes are P for present, A for absent, L for leave, S for sick, HD for half day, LT for late, H for holiday and WO for weekly off. The Excel template uses these codes and you can edit them to match your policy.

Can I use this attendance template for students or volunteers?

Yes. Replace the employee columns with student or volunteer names and adjust the codes. The sheet works for classes, training sessions, clubs and any group where you need to track daily attendance.

Are these attendance templates free and editable?

Yes. Both templates are free to download and fully editable in Microsoft Excel and Word, LibreOffice, Google Sheets and Google Docs. You can add employees, rename departments, change the target percentage and add your company logo.

Download free employee performance report templates: an Excel performance scorecard with weighted ratings, goal results and automatic overall rating, plus a Word performance review form with development plan. Includes rating scale, formulas, review phrases and FAQs.

An employee performance report template gives managers a fair, consistent way to evaluate how well an employee is doing in their role. Instead of writing a review from a blank page, you can use a ready-made performance review form or performance scorecard to rate core competencies, check progress against goals, record strengths and areas for improvement and agree on a development plan.

On this page you can download two free, fully editable templates that work in very different ways: an Employee Performance Scorecard in Excel that calculates weighted competency scores, goal achievement and an automatic overall rating with a radar chart, and an Employee Performance Review Form in Word designed for the face-to-face appraisal meeting, with checkbox ratings, comments, a development plan and signatures. Both suit small businesses, HR teams, team leaders, schools, hospitals and non-profit organizations.

What Is an Employee Performance Report?

An employee performance report is a document that summarizes an employee’s work performance over a defined period, such as a probation period, a quarter or a full year. It usually includes ratings for key competencies (for example job knowledge, quality of work, communication and teamwork), results against agreed goals or KPIs, the manager’s comments, the employee’s own comments and an overall performance rating.

Performance reports are also called performance appraisals, performance evaluations or employee review forms. They are a core HR record: they support decisions on pay rises, promotions, training and confirmation after probation, and they create a written history that can be reviewed in later assessment reports or annual reports.

Why Use an Employee Performance Report Template?

Using the same template for every employee makes reviews faster, fairer and easier to compare. A good performance evaluation template helps you to:

Rate everyone on the same criteria so reviews are consistent across teams and managers.

Reduce bias by using a clear 1–5 rating scale with defined meanings.

Link performance to goals by comparing targets with actual results.

Calculate a fair overall score with weighted competencies instead of a gut feeling.

Have better review conversations with strengths, improvement areas and comments in one place.

Plan development with specific actions, support and target dates.

Keep a documented record signed by the employee and manager for HR files.

Free Employee Performance Report Templates (Editable)

1. Employee Performance Scorecard Template (Excel)

This employee performance scorecard in Excel is built for managers who want an objective, numbers-based evaluation. Enter the employee name, ID, job title, department, manager and review period, then choose the review type (Annual, Mid-Year, Quarterly or Probation) from the drop-down list. Section A lists eight competencies – job knowledge, quality of work, productivity, communication, teamwork, initiative, problem solving and attendance – each with its own weight. Enter a rating from 1 to 5 and the sheet calculates the weighted score automatically, with ratings colored from red to green.

Section B tracks up to six goals or KPIs with a target and an actual result. The template works out the percentage achieved and converts it into a goal rating (for example 110% or more = 5, 100% = 4). Section C combines the competency score (70%) and goals score (30%) into an overall performance score and an automatic rating such as Exceeds Expectations or Needs Improvement. A radar chart shows the employee’s competency profile at a glance, and there is room for manager notes and signatures.

2. Employee Performance Review Form Template (Word)

This performance review form in Word is a printable, conversation-friendly appraisal document. It opens with employee information and a clear rating scale (1 = Unsatisfactory to 5 = Outstanding), followed by a performance ratings table where the manager ticks a box for each competency and adds a short comment. A goals review section lets you mark each objective as Exceeded, Met or Not Met with notes.

The second page focuses on the employee’s growth: key strengths and achievements, areas for improvement, a development plan with actions, support needed and target dates, an overall rating checkbox, a section for the employee’s own comments and signature lines for the employee, manager and HR. It is ideal for filling in during or after the review meeting, either on screen or on paper.

Best for: annual appraisals, probation reviews, small businesses without HR software, and managers who prefer a written, discussion-based review.

What to Include in an Employee Performance Report

A complete performance evaluation report should cover the following sections:

Section

What to include

Why it matters

Employee details

Name, ID, job title, department, manager, review period and type

Identifies the review and the period it covers

Rating scale

Definitions for each rating, e.g. 1–5

Makes ratings consistent between managers

Competency ratings

Scores for skills and behaviors with comments

Shows how the employee does the job

Goals and KPIs

Targets, actual results and % achieved

Shows what the employee delivered

Strengths

Key achievements and positive behaviors

Recognizes good work and motivates

Areas for improvement

Specific gaps with examples

Gives clear, actionable feedback

Development plan

Actions, training, support and target dates

Turns feedback into growth

Overall rating

Final score and rating band

Supports pay, promotion and HR decisions

Comments and signatures

Employee comments, manager and HR sign-off

Confirms the review was discussed and agreed

How to Write an Employee Performance Report (Step by Step)

Prepare before the review. Gather the job description, last review, goals, KPI data, progress reports and examples of work.

Ask for a self-assessment. Let the employee rate themselves first; differences make a good starting point for discussion.

Rate each competency. Use the 1–5 scale and write one specific example for every rating.

Review goals and results. Enter targets and actual results; the Excel scorecard calculates % achieved and the goal rating.

Calculate the overall score. Combine competency and goal scores using your weights (for example 70/30).

Write strengths and improvement areas. Be specific, factual and focused on behavior, not personality.

Agree on a development plan. Set two or three SMART actions with support and target dates.

Hold the review meeting. Discuss the report, listen to the employee and record their comments.

Sign and file. Have the employee, manager and HR sign, then store the report securely.

How the Performance Score Is Calculated

The Excel scorecard uses simple, transparent formulas that you can adjust to your own review policy:

Weighted competency score = Rating × Weight. For example, a rating of 4 for Quality of Work with a 15% weight gives 0.60.

Competency score = sum of all weighted scores (weights must total 100%).

% achieved = Actual ÷ Target. Sales of $132,000 against a $120,000 target = 110%.

Goal rating: 110%+ = 5, 100–109% = 4, 90–99% = 3, 75–89% = 2, below 75% = 1.

Overall score = (Competency score × 70%) + (Average goal rating × 30%).

Overall score

Rating

Typical meaning

4.50 – 5.00

Outstanding

Consistently far exceeds all expectations

3.50 – 4.49

Exceeds Expectations

Often goes beyond what the role requires

2.50 – 3.49

Meets Expectations

Fully and reliably performs the role

1.50 – 2.49

Needs Improvement

Meets some but not all expectations

Below 1.50

Unsatisfactory

Does not meet the basic requirements

Employee Performance Review Phrases (Examples)

Clear, specific comments make a performance report far more useful than ratings alone. Here are some examples you can adapt:

Quality of work: “Delivers accurate, well-organized reports with very few corrections needed.”

Communication: “Keeps stakeholders informed with regular, concise updates.”

Teamwork: “Willingly supports colleagues during peak periods and shares knowledge.”

Initiative: “Identified and fixed a recurring process issue without being asked.”

Needs improvement: “Should plan work further ahead to meet deadlines consistently.”

Development: “Will complete advanced Excel training by the end of Q2 to improve reporting speed.”

Tips for Fair and Effective Performance Reviews

Review performance throughout the year, not only at review time – keep notes or weekly reports.

Base every rating on evidence: results, examples and KPI data.

Avoid recency bias by looking at the whole review period.

Use the same weights and rating scale for everyone in the same role.

Balance positive feedback with constructive improvement points.

Keep development goals specific, realistic and time-bound.

Treat performance reports as confidential HR records.

Performance Scorecard vs Performance Review Form

A performance scorecard is quantitative: it uses weights, ratings and goal results to produce a numerical score, which makes it easy to compare employees and link reviews to bonuses or pay. A performance review form is qualitative and conversational: it captures comments, strengths, improvement areas and the employee’s own views. Many organizations use both – the Excel scorecard to calculate the rating and the Word form as the signed record of the review meeting. For team or department-level results, use a business report or KPI report instead.

Frequently Asked Questions (FAQ)

What is an employee performance report?

An employee performance report is a document that evaluates an employee’s work over a set period. It includes competency ratings, results against goals, strengths, areas for improvement, a development plan and an overall rating, and is signed by the employee and manager.

How do I write a performance report for an employee?

Collect evidence such as goals, KPI results and examples of work, rate each competency on a clear scale, review goal achievement, write specific strengths and improvement points, agree on a development plan, then discuss and sign the report with the employee.

What is a good rating scale for performance reviews?

A 5-point scale is the most common: 1 Unsatisfactory, 2 Needs Improvement, 3 Meets Expectations, 4 Exceeds Expectations and 5 Outstanding. Define each level clearly so all managers use it the same way.

How is an overall performance score calculated?

Multiply each competency rating by its weight and add them up to get a competency score, convert goal achievement into a rating, then combine the two using your chosen weights, for example 70% competencies and 30% goals. The Excel scorecard on this page does this automatically.

How often should employee performance reviews be done?

Most organizations hold a formal annual review with a mid-year check-in. Many now add quarterly reviews or monthly one-to-ones, and new hires usually have a probation review after three to six months.

Can I use these templates for a probation review?
